An answering service is available after office hours and on weekends. A
Cardiologist is always on call and will contact you promptly. If you have
unrelieved chest pain, you should go to your nearest emergency facility. The
answering service is reached after regular office hours, by dialing the main
number at (502) 891-8300.

Referrals and Precertifications
Refer to your insurance policy to know if a Referal or Precertification is
necessary for the office visit or procedure to be scheduled. Please call your
Primary Care Physician prior to your scheduled visit and request a referral
form containing the referral number needed for your visit (if applicable).
Previous Test Reports
If you have undergone recent tests, bringing these reports will save time and
eliminate duplicate testing.

Cardio Vascular Associates is a participating provider with Medicare, Medicaid,
and many managed care plans and provides care for covered patients under
established fee schedules. Please check with your insurance company about your
particular coverage. As a courtesy to our patients, we file all insurance
claims. Patients are reponsible for the payment of all deductibles and
co-payments. Co-payments, required by many insurance companies, are requested
at our registration area prior to appointments. If you do not have insurance
coverage, payment in full is expected at the time of appointment. We gladly
accept cash, personal checks, VISA, and MasterCard and American Express. You
